Letter of Intent

This form takes 2 minutes and ensures that if we meet our funding goal, your students will get to participate in 4-6 free hour-long Adaptive Music Workshops.

If we do not meet our funding goal, your school is not obligated to participate.

After submitting your information, we will send you a copy of your Letter of Intent to sign and send back to us.
